Prior to your appointment

If you are unable to make your appointment, we ask that you cancel ahead of time. Bring your COVID-19 vaccine card or other proof of vaccination to your appointment with you.

You should reschedule your COVID-19 vaccine appointment if 1) you are in isolation due to a positive COIVD-19 test or symptoms of COVID-19; 2) if you have had recent exposure to COVID-19 and are showing symptoms, you should also seek a COVID-19 test. Contact your health care professional to discuss your eligibility before scheduling if you are immune compromised, have questions about your health status and COVID-19 vaccination, or have a history of previous allergic reaction to COVID-19, myocarditis or pericarditis or Multisystem Inflammatory Syndrome with COVID-19. A 15-minute observation period may be recommended following vaccination.

If you have concerns about transportation, please contact us at 218-547-6847.
